"The thermometer reads 102°F (38.9°C). Should I panic? Is the fever dangerous? Should I try to bring it down immediately?"

Few symptoms worry people more than a fever. Parents become anxious when their child feels warm, and adults often rush to take medications at the first sign of a rising temperature. But contrary to popular belief, fever is usually not the illness itself it is a sign that your body is doing exactly what it was designed to do.

In most cases, fever is a natural defense mechanism that helps your immune system fight infections. Understanding why fever occurs can help you know when to manage it at home and when it is time to seek medical care.

What Is a Fever?

A fever is a temporary increase in body temperature, usually caused by the body's response to an infection or inflammation.

The average normal body temperature is often quoted as 37°C (98.6°F), but this is only an average. Normal body temperature varies from person to person and even changes throughout the day. It tends to be lower in the early morning and slightly higher in the evening. Physical activity, eating, hormonal changes, and environmental temperature can also influence body temperature.

In general, a body temperature of 38°C (100.4°F) or higher is considered a fever.

Why Does Our Body Raise Its Temperature?

Imagine your body as a fortress constantly protecting itself from invading bacteria, viruses, fungi, and other harmful microorganisms.

The hypothalamus responds by increasing the body's temperature "set point." Suddenly, your normal body temperature feels too cold to your brain, even though it hasn't changed yet.

To reach this new temperature, your body starts conserving and producing heat.

Why Do We Feel Cold Before a Fever?

One of the most confusing parts of having a fever is feeling cold despite having a high body temperature.

This happens because the hypothalamus has raised the body's target temperature. Until your body reaches this new target, your brain believes you are too cold.

True brain injury from elevated body temperature is generally associated with hyperthermia—conditions such as heat stroke—where the body's temperature regulation fails, rather than with an ordinary infectious fever.

Fever vs. Hyperthermia: They Are Not the Same

Although both involve a high body temperature, fever and hyperthermia occur for completely different reasons.

Fever is a controlled increase in body temperature caused by the hypothalamus in response to illness. The body intentionally raises its temperature as part of the immune response.

Hyperthermia, on the other hand, occurs when the body absorbs or produces more heat than it can lose. This happens in conditions such as heat stroke, where the body's cooling mechanisms fail. Unlike fever, hyperthermia is a true medical emergency and requires immediate treatment.

Should You Always Try to Bring a Fever Down?

Not necessarily.

The main purpose of fever-reducing medications is to improve comfort, not simply to normalize the thermometer reading.

If someone has a mild fever but is otherwise comfortable, well hydrated, and active, treatment may not be necessary.

When fever causes significant discomfort, headache, body aches, or interferes with drinking fluids or sleeping, medications such as paracetamol (acetaminophen) or ibuprofen may help, provided they are used according to medical advice and the recommended dosage.

What About Febrile Seizures?

One of the greatest fears among parents is that fever will cause seizures.

Febrile seizures can occur in some children, usually between 6 months and 5 years of age. They are generally triggered by the body's rapid rise in temperature rather than by the height of the fever itself.

Although witnessing a febrile seizure is frightening, most are brief, resolve on their own, and do not cause brain damage or epilepsy. However, any child experiencing a first seizure should receive prompt medical evaluation to identify the cause and ensure there is no serious underlying condition.

Common Myths About Fever

Many myths about fever continue to circulate despite medical evidence.

Myth: Every fever needs antibiotics. Fact: Antibiotics only treat bacterial infections. Most fevers are caused by viruses, against which antibiotics are ineffective.

Myth: A higher fever always means a more serious illness. Fact: The severity of an illness cannot be judged by temperature alone. Some mild viral infections cause very high fevers, while some serious illnesses may produce only a low-grade fever or none at all.

Myth: You should wrap someone in thick blankets to "sweat the fever out." Fact: While chills may make a person feel cold during the early stages of fever, excessive bundling after the fever has developed can make them more uncomfortable and may hinder normal heat loss.

Myth: Fever itself causes brain damage. Fact: Typical infectious fevers do not cause brain damage. Dangerous elevations in body temperature are more commonly associated with heat stroke and other forms of hyperthermia.
